I ask new patients to complete the following two forms prior to their first appointment with me. The "Client History" form is where you will tell me about yourself - your health, your goals, and your past medical/counseling history. The "Client Disclosure" form is where I tell you more about myself. It describes your right to privacy of your information, and lists under what circumstances I may need to share your information with others.
Please print both of these forms and complete - I'll collect them when we see each other.
